Solving problems with licences
In case of depreciation, damage, change of surname, change of classification, loss, theft of UCM student card, etc. it is necessary to immediately report the incident by phone 033/55 65 196 or by e-mail nora.dostalova@cm.sk.
If you have a non-functioning card, it is not mechanically damaged and 2 years have not elapsed since the card was issued, then deal with the UCM Card Office as a card claim without charge.
Procedure for dealing with a duplicate student ID card:
1. Pay the fee for a duplicate UCM student ID card - by bank transfer.

2. Please fill in the form "Notification of an emergency situation when using a UCM ID card (production of a duplicate, complaint, ...)"
3. Deliver the completed form and payment confirmation in person to the office 6th floor, 2 J.Herda Square, Trnava or by e-mail to: nora.dostalova@ucm.sk, by post to. J. Herdu 2,917 01 Trnava
Transport
If you have used your card with carriers as an electronic wallet (you had some credit on your old card), be sure to indicate this on the form and you will receive a confirmation of the duplicate card in the passes office. Upon confirmation, the transport office of the respective carrier will transfer the remaining credit to your new card.
